The origins of foot reflexology — pressure maps and energy pathways

Foot reflexology draws from two overlapping traditions.

The older is rooted in traditional Chinese medicine and the concept of meridians — energy pathways that run through the body and correspond to specific organs and systems. Stimulating points on the sole of the foot, according to this tradition, influences the connected organ or system.

The Western form of reflexology was systematised in the early twentieth century by Eunice Ingham, an American physiotherapist, who mapped the reflex zones of the foot onto a diagram still used in training today. Her work popularised reflexology across Europe and North America as a standalone therapy distinct from general foot massage.

Thai foot massage is a third tradition — it draws on both the Chinese meridian system and the Ayurvedic concept of the body's energy lines (Sen). It typically includes the lower leg and calf in addition to the sole and toes, making it broader in coverage than either of the other two forms.

At Woravalan, what we practise is closest to traditional Thai foot massage: the full foot, ankle, lower leg and calf, worked with thumbs, fingers and a wooden stick along the reflex points and energy pathways.

What foot reflexology does — and the evidence behind it

The research on reflexology is modest but consistent: it reliablyreduces reported pain and perceived fatigue, improves circulationin the lower leg, and activates the parasympathetic nervous systemin ways similar to other forms of massage. The effects are realeven if the mechanism behind the reflex-zone theory remains debated.

Circulation in the lower leg

Sustained pressure on the sole and calf compresses and releasesthe venous system, encouraging blood flow back toward the heart. This is particularly effective after long flights, long walks orextended periods of standing — all common in Phuket's climate andterrain.

Plantar fascia and sole tension

The plantar fascia — the band of tissue running along the bottomof the foot — shortens and tightens with walking, particularly onhard surfaces. Thumb pressure along the arch and heel releasesthis tension and reduces the ache that most people simply acceptas part of a day on their feet.

Lower leg and calf

Most foot massage sessions stop at the ankle. Ours continue upthrough the gastrocnemius and soleus — the two main calf muscles— which carry significant tension after any amount of walkingor standing. Including the calf changes the session from asurface treatment to something that addresses the whole load-bearingchain from foot to knee.

Nervous system response

The density of nerve endings in the sole of the foot is among thehighest in the body. Sustained rhythmic pressure activates theparasympathetic nervous system — the same response that occursduring full-body massage, but accessible through a single focusedarea without the need to undress or lie on a table.

What happens during a foot massage session at Woravalan

You sit in a reclining chair in the private suite. The door closes.

Woravalan begins with a warm soak for the feet — a few minutes to soften the skin and begin the circulation response. She then works the sole with her thumbs, following the reflex map from the heel through the arch to the ball and toes. Each zone receives several passes at varying depths. If a point is particularly tender, she stays with it longer.

After the sole she moves to the ankle — rotating it through its full range of motion and working around the joint with her thumbs. Then the lower leg: long strokes up the calf with both hands, followed by deeper thumb work along the gastrocnemius and the tibialis anterior. The session ends with light effleurage strokes from the ankle to the knee on both legs.

A 45-minute session covers both feet and lower legs at a thorough depth. If you want more time in specific areas — or if your feet need more attention than usual after a long walk — a 60-minute session allows for that.

After the session you sit for a moment before standing. The feet often feel noticeably lighter and slightly warm. That is the circulation response working.
