Why neck and shoulder massage Phuket works — what is actually holding the tension

The upper trapezius — the muscle that runs from the base of the skull to the outer edge of the shoulder — is among the most chronically overloaded muscles in the human body. It carries the weight of the head (approximately 5–6 kg) through its attachment to the cervical spine, stabilises the shoulder blade during arm movement, and responds to psychological stress by increasing its baseline tone.

When the head moves forward from its neutral position — as it does when looking at a screen, a phone, or sleeping in an awkward position — the effective load on the upper trapezius and the deep cervical extensors increases dramatically. Most people who use screens for more than four hours a day have a head position that is chronically forward of neutral.

The suboccipital muscles at the base of the skull are typically the most tender point: when pressed, they often refer pain forward into the temple and behind the eye — producing the tension headache that most people attribute to stress or dehydration.

This is the structural reality behind neck and shoulder tension. It responds well to targeted massage — but only if the right muscles are addressed in the right sequence.

Desk work and screen time

The combination of forward head posture, rounded shoulders and sustained static muscle load of desk work produces a predictable pattern: tight upper trapezius, weak deep neck flexors, restricted thoracic mobility. A 60-minute focused session on the neck, upper back and shoulders addresses the tight half of this equation. Most clients notice improvement in posture and range of motion within 24 hours.

Post-flight stiffness

Long-haul flights produce neck stiffness through a combination of sustained awkward head position, low cabin humidity and the immobility that makes any position rigid over many hours. The cervical spine and the surrounding musculature respond well to the combination of heat, sustained compression and passive cervical rotation that a neck and shoulder session provides. Book within 24 hours of landing for best results.

Frozen shoulder

Frozen shoulder — adhesive capsulitis — is a condition in which the shoulder joint capsule becomes inflamed and contracted, progressively restricting movement. Massage cannot address the capsule directly, but it significantly reduces the protective muscle spasm that surrounds the joint and compounds the restriction. Regular work on the surrounding musculature — deltoid, rotator cuff, pectorals, biceps tendon attachment — reduces pain and supports the range of motion recovery process. Tell your therapist if you have frozen shoulder so she can adapt the pressure and avoid aggravating the joint capsule directly.

Tension headache

As described in the anatomy section: most tension headaches originate in the suboccipital muscles and upper trapezius. A focused neck and shoulder session works both the source muscles and the referral pathway. Most clients with active tension headaches notice relief during the session itself. What happens during a neck and shoulder massage at Woravalan

You arrive and your therapist asks specifically about your
complaint: where, when, what makes it worse. Two minutes.

You lie face-down on the table, a bolster under your ankles,
your face in the headrest. Your therapist begins with broad
effleurage strokes across the upper back to warm the tissue
before the deeper work. When the muscle is warm, the work begins with the upper trapezius: sustained pressure along the
muscle belly from the shoulder to the base of the skull,
pausing at trigger points and holding until the referral
pattern subsides.

The levator scapulae is worked along the inner border of
the shoulder blade, then turns to the thoracic spine and
the rhomboids — the muscles between the shoulder blades
that typically ache from prolonged forward rounding.

The session finishes at the subocciput: sustained thumb pressure
along the ridge at the base of the skull, and passive
rotation of the cervical spine — holding each rotation
for several breaths. Most clients experience a significant
release here.

You turn onto your back for the final phase: the
sternocleidomastoid along the side of the neck, the
scalene muscles and the jaw, which carries significant
tension in people who clench or grind.

A 45-minute session covers the core area thoroughly.
60 minutes allows more time at specific trigger points
and the cervical rotation work.
