Why pregnancy massage — what changes in your body and what massage addresses

Pregnancy places a significant and progressive mechanical load on the body. The centre of gravity shifts forward as the belly grows, creating compensatory tension in the lower back and hip flexors. The hormones of pregnancy — particularly relaxin — increase joint laxity, which can lead to instability and pain in the sacroiliac joint, the pubic symphysis and the hips. The additional weight carried in the front of the body shortens and tightens the hip flexors and compresses the lumbar spine. The shoulders round forward as the chest accommodates the changes in posture.

Alongside these structural changes, pregnancy increases blood volume by up to fifty percent and slows venous return from the lower limbs — which is why swollen ankles and legs are almost universal by the third trimester. The circulatory load is accompanied by elevated cortisol and the general nervous system activation that comes with carrying a growing pregnancy.

Prenatal massage addresses all of these changes directly. It is not a luxury — it is a therapeutic response to a set of physical demands that are specific to pregnancy.

The evidence base for prenatal massage is well-established: regular massage during pregnancy reduces reported lower back pain, improves sleep quality, reduces cortisol and increases serotonin and dopamine, and is associated with lower rates of premature birth in some studies.

What is different about prenatal massage — the adaptations that make it safe

Prenatal massage is not a standard massage performed on a pregnant woman. It requires specific adaptations to positioning, pressure and the areas treated. These adaptations exist for clear physiological reasons — not out of excessive caution.

Positioning — why lying face-down is not possible

After the first trimester, lying face-down places pressure on the uterus and the inferior vena cava — the large vein that returns blood from the lower body to the heart. Compression of the vena cava can cause a drop in blood pressure, dizziness and reduced flow to the foetus.

Prenatal massage uses a side-lying position throughout, supported by bolsters: one under the head, one supporting the belly, one between the knees to maintain spinal alignment. This position is comfortable, stable and allows the therapist access to the back, hips, legs and shoulders. Some therapists use a specialised table with a cutout — we use full bolster support, which is equally effective and more adaptable across different stages of pregnancy.

Pressure — what changes and what does not

Firm pressure on the lower back and abdomen is avoided. The uterine ligaments — which attach the uterus to the pelvic wall — run near the inner thigh, and strong pressure in this area is avoided. Certain acupressure points traditionally associated with stimulating uterine contractions are avoided.

Everything else — the back, shoulders, hips, legs, feet and neck — can be addressed with therapeutic pressure calibrated to your comfort and your stage of pregnancy. Lighter touch is not necessary for the whole body; only specific areas require specific care.

First trimester — why we recommend waiting

The first trimester carries the highest risk of miscarriage — most of which occur for genetic reasons unrelated to massage. However, because the risk exists and because massage increases circulation and hormonal activity, most certified prenatal massage therapists and the organisations that train them recommend waiting until the second trimester before booking.

What a prenatal massage session at Woravalan looks like

You arrive and your therapist asks about your pregnancy — how many weeks, whether you have had any complications, what your GP or OB has said, and what is bothering you most. This conversation takes about five minutes and shapes the entire session.

You change into loose comfortable clothing — you can stay partially dressed if you prefer. Your therapist positions the bolsters: one under your head, one cradling your belly, one between your knees. You lie on your left side — this is the preferred position because it optimises blood flow to the kidneys and the uterus.

The session focuses on the areas that carry the most load:

Lower back and sacrum: the area that typically hurts most. Your therapist works with careful pressure along the paravertebral muscles and the gluteal muscles — avoiding direct pressure on the sacroiliac joint if it is unstable.

Hips and IT band: the hip flexors tighten as the pelvis tilts forward. Your therapist works the hip rotators and the outer thigh, releasing the tension that contributes to the ache in the outer hip that many pregnant women know.

Shoulders and neck: the rounding of the upper back that comes with pregnancy and breast changes. Long strokes and compression along the upper trapezius and levator scapulae.

Legs and ankles: gentle effleurage strokes that support venous and lymphatic return from the lower limbs, reducing the swelling that accumulates in the heat.

The session ends with a few quiet minutes before you sit up. Your therapist does not rush this.

After the session, drink water and move gently for the rest of the day. Most clients sleep better the night after.

The bolsters — proper support for every stage

Prenatal massage requires proper positioning equipment. Woravalan has a full set of bolsters for side-lying support: a head bolster, a belly bolster sized for different stages of pregnancy, and a knee bolster for spinal alignment. The setup is adjusted at each session as your body changes.

You do not need to bring anything. Tell us how many weeks you are when you book and Woravalan sets up accordingly.

A note on equipment:
Some studios use a specialised pregnancy massage table with a central cutout that allows face-down lying. Woravalan uses side-lying bolster support throughout instead — this is the preferred approach of most certified prenatal massage organisations, because it keeps the spine in proper alignment and remains appropriate at all stages of pregnancy, including the third trimester when the cutout tables become difficult to use.

The suite — private, warm and set up for this treatment

Woravalan has two private suites. Prenatal massage is performed in a private room — door closed, nobody walking through. The table is positioned with clear access on both sides. The temperature is warm. The lighting is adjustable.
