Massage for Headaches and Migraines: Which Types Help Most

Headaches and migraines are among the most debilitating recurring symptoms that people manage, and they are also among the conditions that respond most variably to treatment. The evidence for massage as an intervention depends significantly on the type of headache involved and the timing of the treatment relative to the headache cycle.

Tension-Type Headaches

Tension-type headaches — the most common form, characterised by a band-like tightness around the head, pressure behind the eyes, and often originating in the neck and suboccipital region — are directly amenable to massage treatment. The muscular tension that generates the headache is addressable through precisely the techniques that form the upper body and neck component of a Thai massage session. Many guests with regular tension headaches find that a focused neck and shoulder session either eliminates an existing headache or prevents the next one.

Cervicogenic Headaches

Cervicogenic headaches originate in the cervical spine — typically at C2 or C3 — and refer pain forward and upward across the scalp. They are caused by joint dysfunction and muscular restriction in the upper cervical region, and they respond to treatment of the underlying cervical pattern. Thai massage's detailed cervical and posterior neck work directly addresses the primary pain generator in these headaches.

Migraines: More Caution Required

Migraine is a neurological condition rather than a musculoskeletal one, and the approach to massage for migraine requires more caution. During an active migraine episode, massage is generally contraindicated — the increased circulation and the sensory input of pressure can worsen symptoms. Between episodes, regular massage that reduces baseline sympathetic nervous system activation and cortisol may reduce migraine frequency. This should be discussed with a neurologist in the context of an existing migraine management plan.
