Can Thai Massage Help Frozen Shoulder?

Frozen shoulder — clinically known as adhesive capsulitis — is one of the most painful and functionally limiting conditions that affects middle-aged adults. It involves the gradual thickening and contraction of the shoulder joint capsule, which reduces range of motion in all planes and produces significant pain particularly at the extremes of movement. The condition typically progresses through three phases over 12 to 24 months: a freezing phase, a frozen phase, and a thawing phase.

Where Thai Massage Can Help

During the frozen and early thawing phases, the focus of Thai massage work around the shoulder is not on forcing the joint back into its full range — this is counterproductive and painful — but on addressing the significant secondary tension that develops in the surrounding musculature. The rotator cuff muscles, the trapezius, the rhomboids, and the posterior arm muscles all develop chronic tension as the body compensates for the restricted shoulder, and this compensatory tension is highly amenable to Thai massage techniques.

The Arm and Shoulder Sequence

Traditional Thai massage includes a detailed arm and shoulder sequence that works the arm from the wrist through the forearm, bicep and tricep, into the posterior shoulder, and through the upper trapezius and cervical musculature. This sequence can be adapted for frozen shoulder by working with the arm in a supported position within the available range, applying deep pressure along the muscle bellies without taking the shoulder to its end range, and using gentle passive mobilisation within the pain-free arc.

Before any session where frozen shoulder is a factor, the therapist needs to know which phase you are in, which movements are most restricted, and what the current pain level is at rest versus with movement.
